Logo Hôpital MontfortThe project is entitled Co-constructing the Medication Reconciliation Process: Collaborative Research on Training based on Partners’ Needs Recognition and Contributions. It aims to experiment with a method that optimizes medication reconciliation through a richer collaboration that involves all health professionals in addition to patients. This is a pilot project that is being carried out at Montfort Hospital with stakeholders from 3 units/departments.

Results will make it possible, on the one hand, to optimize what is already being done in terms of medication reconciliation and on the other hand to amass preliminary data that will be used to submit a grant application to CIHR. The project is financed by Savoir Montfort with a $ 80,000 budget for 2017-2019.
